All forms should be completed at your earliest convenience, no later than August 5, 2024.

Preparticipation Physical Evaluation

The Preparticipation Physical Evaluation must be printed, completed by a physician and turned in to Mrs. Smith/Mrs. McCoy/Mr. Reinzan by the 1st day of Percussion  Camp - Monday, July 14, 2025 (percussionists). Winds (brass & woodwinds) must have their physical forms turned in by Monday July 21, 2025
